Personal History

Before you're allowed to input Lithuania, you will need to fill out a Lithuania Registration Form. This is needed by all foreign residents of Lithuania, even though they may not yet have a resident visa. The Ministry of Foreign Affairs in Lithuania will make it possible for you to acquire a visa so as to travel to Lithuania, but you will still need to fill out the enrollment form before you may travel. You can still use your current passport as a valid form of identification

You must also indicate whether you need a medical insurance card. You will be required to give information such as name, contact information, telephone number, phone number, date of birth, passport expires, and payment methods. You'll also be required to list any other states which you travel to on your enrollment form, like the U.S., U.K., France, Germany, and Italy. A negative diagnosis on your health authorities' records can prevent you from getting a health travel insurance plan in Lithuania.

Your enrollment form may ask you to list all of your vaccinations, such as your first and third series of immunizations. If you've gotten any other vaccinations as your first visit to the vaccination center in Lithuanian capital Vilnius, you will need to list these also. Your enrollment form will define if your vaccinations need to be revived, and you will have to indicate whether or not you wish to continue your treatments at the centres. For those that did not get a vaccination certificate at the time of your first visit to the vaccination center in Vilnius, you will still have to indicate these on your registration form.

List all of your personal items. These include: passport, vaccination card, airline tickets, and any prescriptions. You will also have to list all your personal items that are in your luggage or in your car. It's critical to indicate whether any of your prescription medications have been in your possession, such as: vitamins, aspirin, and blood pressure medications.

List all your personal illnesses and conditions. You will need to describe if your disease or condition is not yet diagnosed. If it isn't a contagious disease that can spread readily, you will also have to describe if it has a specific name (i.e. strep throat). The only contagious diseases that need an entry into the vaccination centre's database are the ones that are automatically contracted through contact, inhalation, or blood.

List your marriage/divorce details. The only entry necessary for this part of your Lithuania Registration Type is your married name. It's likewise a good idea to indicate whether you were previously divorced or still currently being wed. The health authorities will also have to know the names of any children or people that are living with you. Be sure to record the legal titles of these individuals as well, for example, town and town they live in. Each these details must be in your enrollment form for Lithuania.
